Consumer Product Safety Commission to report an injury or file a complaint about an unsafe consumer product.
 
National Highway Transportation Safety Administration  to report a vehicle, tire, child restraint or vehicle equipment safety problem

U.S. Food and Drug Administration to report problems with foods, medicines, cosmetics and medical devices (The CPSC has authority for child-resistant packaging)
 
Federal Trade Commission for complaints about business practices
